Improving physician documentation specificity
Differentiate between tobacco use/abuse or dependence (i.e., addiction to tobacco products caused by the drug nicotine).
Document type of tobacco product (e.g., cigarettes, chewing tobacco, or cigars).
Differentiate between patients who no longer smoke and those that do. Instead of history of smoking, use former smoker.
Document if patient exposed to second-hand smoke.
